Join our Sales team as a Clinical Liaison! The Clinical Liaison is responsible for the data collection and coordination of admissions for referrals from inpatient acute care and sub-acute care settings, including internal residents of the Erickson Living community. On-site review is conducted whenever possible to facilitate relationship building with professional referral sources, as well as to expedite referral management. The Clinical Liaison will collaborate with Physicians, Continuing Care Sales/Admissions Staff, Hospital Case Managers, Social Workers, and/or Discharge Planners, to facilitate the admissions/transfer process. The Clinical Liaison assists in marketing outreach to referral professionals in hospitals and physician offices. The Clinical Liaison is an active participant with the Healthcare Sales Team in meeting and/or exceeding census and revenue goals
. How You Will Make An Impact - Collaborates with hospital discharge planners, case managers, the physician, Director of Nursing, and Administrator of any medically complex resident seeking admission.
- Monitors and supports residents and families who are at the hospital or are outplaced at alternate care facilities and provides support as they navigate back to campus.
- Coordination of move-in dates/admissions and transfer of residents throughout the continuum to SNF/LTC.
- Responsible for onsite visits and clinical review of residents’ needs and care, including Holistic Assessments.
- Supports all aspects of the Continuing Care team’s processes for Continuing Care as well as supporting an integrated healthcare approach to care offered on campus.
- Develop and nurture resident and team relationships to assure residents care needs are met and to ensure quality care and service
- Supports Continuing Care Sales team with outreach efforts and networking inside and outside the community.
- Detail-oriented with the ability to handle multiple tasks simultaneously.
- Excellent organizational and time management skills
- Ability to promote and maintain positive and professional working relationships at all times

What You Will Need - Minimum of 3 years of clinical experience in an acute hospital setting preferred
- Liaison experience or healthcare marketing background preferred
- Familiarity with acute hospitals, rehabilitation hospitals, sub-acute rehabilitation facilities, and the local healthcare market preferred
- Must maintain valid state driving license
